Resolution
This type of issue surfaces as dependent databases, such as linked dSources, have not been upgraded in the Delphix Appliance and Delphix will continue to monitor the health of the former Oracle Home.
After the initial in-place/existing upgrade of the Oracle Home has completed and the new Oracle Home/Version has been detected by performing an Environment Refresh, upgrade each dSource using the procedure located in the Delphix Documentation under the section "Upgrading dSources after an Oracle Upgrade" and once completed, perform an additional Environment Refresh.
