Problem

During provisioning the following error is presented

dbgc_init_all failed with ORA-48141
RMAN-00571: ===========================================================
RMAN-00569: =============== ERROR MESSAGE STACK FOLLOWS ===============
RMAN-00571: ===========================================================
RMAN-12001: could not open channel default
RMAN-10008: could not create channel context
RMAN-10013: error initializing PL/SQL

Cause

This issue is caused by a known Oracle bug, documented in Oracle Note 2040126.1. Please review the Oracle note for a full description of the issue and links to patches for the bug.

Workaround

There are two options to work around this issue

1. Provision the database using the same OS user that owns the Oracle instance.

2. Change the permissions of $ADR_BASE/diag/plsql directory to 777.
